Disclosure of Relevant Financial Relationship Form (2017-2018): Everybody that is in the position to influence the content of the activity (incl. Planning Committee, speakers, etc.) will need to submit disclosure information prior to the activity date to ensure that our CME activities are independent from commercial interest.
Summary of Disclosures Form (2017-2018): This is the summary of all submitted disclosure info which the CME department will put together prior to the activity date. This form will need to be distributed to the learners prior to the activity start time – this can be done through print-outs in the welcome folder or via email, etc.
