Welcome to the Greenville Health System Health Sciences Center. We’re thankful to have you as a member of our team. As you settle into your role, it’s normal to have a lot of questions. We have compiled a variety of tools and resources to support you through your first days and weeks on the job.
To begin working at Greenville Health System (GHS) campus, you will be required to complete several steps within the faculty onboarding process. To start this process, you will receive an e-mail from the Office of Faculty Affairs & Professional development.
Required Information for Completing the Compliance Checklist

You will receive an e-mail from the Office of Faculty Affairs & Professional Development that provides you with instructions on becoming a Faculty member of the GHS Health Sciences Center. - Health Record and Information Form

Most of you will have your immunizations completed by your home institutions Health Services Office before coming onto the GHS Campus. For health screening and clearance purposes we will ask you to bring these immunization records with you to your immunization appointment. You can use the health screening services at your home institution or at CHOS (Center of Health and Occupational Services) right off the Greenville Health System campus. A map is provided here. The Office of Faculty Affairs & Professional Development will provide you with the Medical Treatment Authorization form.

Important Information for Choosing the Center of Health and Occupational Services (CHOS)
If you have chosen to use the Center of Health and Occupational Services (CHOS) to verify that you have received the proper immunizations, you will take the Medical Treatment Authorization and your existing immunization documents with you to your appointment. It is advised that you start this process at least 3 weeks before your official start date as some tests will require several trips.Health Record and Information Form and supporting documentation- This form outlines the immunization and TB test requirements for all faculty in GHS facilities. Please note that GHS requires a 2-step TB Test, which will require 4 trips spaced several days apart to a healthcare provider. On your first visit, you will receive your first TB test, return to the provider as directed to have it read, and will return several days later to receive a 2nd TB test, and then will return as directed to have it read. If you have had a TB test in the past 12 months and can provide the test results (negative test results only), you will only need one additional TB test. Please note that this process may take two or more weeks to complete all requirements. You will need to provide copies of all immunizations and TB test results in addition to the completed Health Record and Information Form.
- The GHS Center for Health and Occupational Services (CHOS) will provide any immunizations, titers, and/or TB Tests for faculty, scholars and research associates. Contact CHOS to schedule your appointment and be sure to bring your immunization records with you to the appointment. You will also need to bring the Medical Treatment Authorization Form with you to the appointment, identifying you as Health Sciences Center faculty or HSC faculty/research associate.

You will receive this as part of your instruction e-mail from the Office of Faculty Affairs & Professional Development. - HealthStream
HealthStream is the GHS-wide learning management system that provides important healthcare modules required to begin your experience at GHS (HIPAA, infection control, patient rights, etc.) The Office of Faculty Affairs & Professional Development will contact you once your learning account has been created. - The Electronic Health Record (EHR) Authorized User Form
If this is necessary, the Office of Faculty Affairs and Professional Development wills send this to you
for your Clemson/Furman/USC authorized supervisor/faculty member to sign and return. - Badges
Once Steps 1, 2, and 3 are completed a badge request will be submitted and our office will inform you when and where to pick this up. Badge Information
Faculty and research associates will receive a GHS photo badge with their name and title. Titles will read as “Health Sciences Center Faculty” or “Health Sciences Center Research Associate.” Some badges may have access to specific areas pertinent to the area in which the individual will be working. These requests should be made by an administrator from the department in which the faculty or researcher will be working.Faculty and research associates need to wear their GHS-issued identification badge at all times when in a GHS facility in their role as faculty or research associate.

HSC Office Faculty Affairs & Professional Development is located at the Greenville Health System Campus on 701 Grove Rd, Health Sciences Administration Building (HSAB) room 209-Next to the USC Medical School.

Hours of Operation: Monday, Tuesday, Thursday, & Friday 8:30am – 4:30pm Wednesday 7:00am – 3:00pm. The office usually closes for lunch 11:30 – 12:00.
